This form is to register your team with our league. NOTE: Each individual player on your team must register as a player to play in this league. When you fill out your individual player registration be sure to designate what team you are registering with. Team "Cores" can only consist of 5 players all other players must register as individuals. ANY ONE PLAYER CAN ONLY BE PART OF ONE TEAM IN THIS LEAGUE.
